Assessment Practice Certificate - this means any diagnosis made is lifelong and can be used to access disabled student’s allowance, (DSA), in Higher Education.

Cost of Assessment:
Dyscalculia or Dyslexia Screener Report - £200.00
Including a series of questionnaires to provide background information, specific tests to ascertain strengths and weaknesses, and an indication of the likelihood of dyscalculia, and a concise report of this information.
Dyscalculia Full Diagnostic Assessment: £650.00
What do I get for that?
- A series of questionnaires to build a comprehensive set of background information.
- A full diagnostic assessment, which will take around 4 hours.
- A lengthy diagnostic report which will detail the results and what they mean, strengths and weaknesses, an appraisal of the background information, and an informed set of recommendations.
- A summary of the report for ease of use.
- A diagnosis of dyslexia or not, which is lifelong and can be used for DSA
- A report within 2 weeks of the assessment date.
- For any data to be stored securely in line with GDPR rules and regulations, and for the legally required length of time.
Dyslexia Full Diagnostic Assessment: £550.00
What do I get for that?
- A series of questionnaires to build a comprehensive set of background information.
- A full diagnostic assessment, which will take around 4 hours.
- A lengthy diagnostic report which will detail the results and what they mean, strengths and weaknesses, an appraisal of the background information, and an informed set of recommendations.
- A summary of the report for ease of use.
- A diagnosis of dyscalculia or not, which is lifelong and can be used for DSA
- A report within 2 weeks of the assessment date.
- For any data to be stored securely in line with GDPR rules and regulations, and for the legally required length of time.
Combined Dyscalculia and Dyslexia diagnostic assessment - £850.00
What do I get?
- The above, combined, but the assessment may take place over two sittings.
- A diagnosis of dyslexia, dyscalculia, both, or neither.
How and when do you pay?
A £50.00 fee is payable after receipt of the consent form and before despatch of the initial questionnaires. This is non-refundable as it is the cost of this part of the assessment process.
The remainder of the fee, in its entirety, is due upon arrangement of an assessment date(s)..
Cancellation Policy:
Cancellation by the client or main contact:
- All changes and cancellations must be notified, in writing, to: info@the-adaptive-education-hub.com
- Cancellations or rescheduling within 21 days of the assessment date = 50% of the fee.
- Cancellations or rescheduling within 3 days of the assessment date = 100% of the fee.
- Cancellations over 21 days before the assessment incur a £50.00 administration fee.
Cancellation by the assessor:
In the event of any emergency resulting in the cancellation of an assessment, the assessor will endeavour to give clients a minimum of 24 hours’ notice and to reschedule the assessment.
*Please note that it is the client’s responsibility to provide the assessor with an emergency contact number at which they will normally be available.
Booking Diagnostic assessment:
- After an initial contact you will receive a booking form and permission letter (to be completed by both parents or guardians in the case of minors).
- Upon completion of that form a set of background questionnaires will be electronically forwarded to you.
- You are required to complete the forms, giving as much information as you can,
- You should forward the school questionnaire for them to complete.
- Your child should have had a sight and hearing test within the previous two years. If this has not been done, you will be required to book and attend an eyesight test prior to the assessment date.
ONCE THE ABOVE IS COMPLETED AND RETURNED.
- An assessment date will be booked, and at that point, the deposit becomes due. (The Adaptive Education Hub reserves the right to change the assessment date in exceptional circumstances, e.g., illness.)
- On the assessment day, a quiet space will be required where the assessor can sit opposite the pupil without interruption. A power point, close by, is useful.
- The assessments will take around 3-4 hours, and the pupil will be given a break if necessary, and can obviously take toilet breaks.
- It is vital that the assessment situation is as normal as possible, so any fiddle toys, overlays, glasses, hearing aids, etc, should be available along with whatever the pupil likes to write with.
- Upon completion of the assessment, you can expect a full report within 2 weeks. (If this is to be different, you will be informed on the day.)
Privacy Policy:
The Adaptive Education Hub is committed to protecting the privacy of individuals. Accordingly, all personal data collected will be subject to our privacy policy, which can be viewed on request. Following the assessment, the compilation of the final assessment report and final approval have been given around the content of the report, all client questionnaires, all raw data test sheets, and all other personal data will be permanently deleted/destroyed. If no response regarding scheduling the assessment is received, all details will be permanently deleted after 7 days due to the personal information that the booking details contain
Your final assessment report will be held by The Adaptive Education Hub for 6 years (Adults), or until the child turns 25 [within a customer record file]. During this time, you will be able to ask for an electronic copy of the report. Please note there may be an administration charge for this. We would therefore strongly recommend that you keep a copy of your report securely. After this time, the Adaptive Education Hub will not be able to supply you with a copy of your report.
